Sleep Diaries

If possible, before your first appointment, please complete the sleep diary below that applies to you and bring it along to your appointment.

Sleep diaries can be a helpful way to gain insight into sleep patterns, notice trends, and observe changes over time. We don’t recommend tracking sleep long-term, as this can sometimes increase worry or over-focus on sleep, but a short period (7-14 days) of recording can be very informative.

Record times as accurately as you comfortably can — there’s no need to watch the clock overnight to fill in your diary.

Questionnaires

Prior to, during or after your appointment you may be asked to complete one of the following questionnaires below. Questionnaire results do not constitute a diagnosis. They are used as a screening tool to gather more information and may indicate the need for additional assessment or testing.

Insomnia Screeners
